You can also find both forms here at www.poos4u.net  ,copy them to your word program ,fill in and e-mail them to us .

When contacting us via e-mail please put in subject line the type puppy ...example "maltepoo, maltipoo or yorkiepoo or other "  so that we open your e-mail .There are so many viruses going around that it is much easier for us if we know what you are seeking ,we do not always open e-mails that have no subject line .Thanks !
